Why might a change not occur after proofreading? There are few to no changes when proofreading because, the writer might not be sure if the change made would be correct or not. Also, the writer or proofreading could have missed the mistake all in whole. If you are proofreading, you should always be willing to edit or make a change. Otherwise, you are ready to publish and you are just review your written material. There are always room for improvement (in anything), even professionals.
